Job Title: Fitness Specialist Company: Location: Teec Nos Pos, AZ Description: DUTIES:   This position is located in the Four Corners Wellness Center at the Community Health Services Branch, Four Corners Regional Health Center (4CRHC), Shiprock Service Unit.  Incumbent primarily serves the needs of four chapter areas within the greater Shiprock Service Unit, Sweetwater, Red Mesa, Mexican Water, and Teecnospos, Arizona.  Serves as Fitness Specialist, providing physical activity guidance, healthy lifestyle consultation and specific fitness program/equipment orientation and instruction for the Wellness Center clientele, and maintenance of fitness equipment.  Works with a high degree of independence, must be able to exercise independent professional judgement in designing and carrying out an exercise plan.  Serves as fitness specialist and healthy lifestyle educator providing these services to individuals, groups, and families within the 4CRHC.  Conducts health screening assessments for all clients to determine if further medical evaluation is needed prior to beginning a fitness program.  Designs fitness plans consistent with cultural, social, and physiological requirements.  Monitors and evaluates the progress of clientele independently, able to make changes in exercise plans and intervention approaches in accordance with exercise science practice.  Facilitates teaching and training of clients, staff, and community members to expand services and increase public awareness of diabetes control and prevention, as well as prevention and treatment of other chronic diseases via physical activity.  Exercise demonstrations will be a part of this teaching.  Supports 4CRHC's efforts to develop and provide healthy lifestyle group training programs such as Native Lifestyle Balance.  Provides consultation to professional and paraprofessional staff and other programs at the local, tribal, state, and federal levels regarding the influence of physical activity in mitigating chronic disease such as diabetes.  Participates in routine records maintenance in compliance with the Privacy Act, HIPAA and Indian Health Service policies.  Assists with achievement of other Health Promotion objectives as needed.  Utilizes latest health, fitness, scientific and medical knowledge in providing and evaluating health and fitness services.  Consults with other members of the 4CRHC; Rehabilitation Services, Diabetes Education Program, Health Promotion, Medical Providers, and Pharmacy staff regarding clinical application and services.
